Dear Ma'am,
I am writing to inform you that I met with an accident on my way back home on the 12th of August.
I have been advised to take rest for 5 days (i. e. from 13th to 17th of August 2016). I have enclosed the doctor's medical certificate.
I would request you to consider my request and grant me leave for 5 days. I would be available over the phone during this time should anything need my urgent attention.
